I’m unable to apply automatic numbering to other than left-aligned rows. This is problematic when one needs to create a centered “Chapter #” heading, for example.
Is this intentional, a bug, my stupidity…?

This is a very old bug unfortunately. If you want to have automatic updating chapter headings, a workaround would be to insert a bunch of tab characters before “Chapter” in the prefix field. You can insert those with option-tab. Obviously they won’t auto center but you’ll have some level of control instead of just having them flush on the left.
