Omnifocus says it's synced but it's not

I’ve been having a problem where Omnifocus on my Mac and my iPhone won’t stay in sync, even though they both claim to be in sync. I can look in Settings and see that they were both synced within the last minute, but they show different data (tasks checked off on my phone are not checked off on Mac, and vice versa).

I’m running Omnifocus 4.3.2 on a MacBook Pro running Mac OS Sonoma 14.6.1. iPhone 13 mini running iOS 17.5.1.

Update: I found that when I click “Rebuild Database” the correct info will appear on my Mac. But I have to do this once every few days.
